Coming home should feel like relief, not worry.

After a stay in hospital, even familiar routines can feel harder.
Getting washed and dressed, making a meal or managing new medication can take more energy than before, and families often worry about how things will go once they're back home.
Our hospital discharge support gives your loved one a steady hand through those first days and weeks. We can help with the everyday tasks, keep an eye on how they're doing and let you know if anything changes, so recovery can happen in the place they feel most comfortable.
It's short-term support by design, but if it turns out more help is needed, we'll talk it through with you and adjust the care plan.

Not sure what you need? That’s completely fine. Let’s start with a conversation.
We're a local family, caring for yours.
Our family's caring story began in the 1990s. Nightingales Care was established in Bognor Regis in 2009, carrying forward the same belief that people deserve kindness, dignity and to feel truly known.
We're independent, not a national franchise. That means a local team you can speak to, and care shaped around the person rather than a standard package.
Personal, not one-size-fits-all
We start with the person: what they enjoy, what matters to them and how they want to live.
Familiar faces you can trust
We know that trust takes time, so continuity and relationships are an important part of how we care.
Here for families as well
We're available to talk through concerns, explain the options and review care as circumstances change.
Short-term support today. More if it's needed tomorrow.
For many families, hospital discharge support is the first step. It might be a few weeks of extra help while strength returns, and if more is needed it can grow into regular home care or live-in care.
There's no commitment to more than you want. We'll talk through what would help now, put together a personalised plan, and review it with you as your situation changes.

A LITTLE CLARITY
Your questions,
answered.
There’s no such thing as a small question when it comes to choosing care.
How soon can care start after a hospital stay?
Straight away, once we've completed a care assessment. Call us as early as you can, ideally before the discharge date is confirmed, and we'll arrange the assessment around it so care is ready when your loved one gets home.
How long does hospital discharge support last?
As long as it's needed. For many people it's a few weeks of extra help while they regain strength; for others it becomes ongoing home care. We review the plan with you as things improve.
Can you work from the hospital's discharge plan?
Yes. We'll look at any discharge notes, medication changes and advice from the hospital team, and build the care plan around them.
Do you help with personal care and medication?
Yes. Our carers can help with washing, dressing, continence care and medication prompting, all agreed in the care plan.
What if they need more support than expected?
We'll talk it through with you straight away. Visits can be increased, and we can discuss live-in care, or overnight support subject to availability.
Is hospital discharge care funded?
Sometimes. Short-term support after hospital may be arranged through the NHS or West Sussex County Council's reablement services, so ask the hospital discharge team. If you're arranging care privately, we'll explain costs clearly before anything starts.
Which areas do you cover?
Bognor Regis, Chichester and nearby villages including Aldwick, Felpham, Pagham, Middleton-on-Sea, Bersted and Rose Green.
NO PRESSURE. NO RUSH.
We’ll take it
one step at a time.
You don’t need to have everything worked out before you get in touch. We’ll listen first.
